SILC Support Coordination Services

Since September 2016, SILC has supported people and families under the NDIS to coordinate services and create independent housing and support pathways that work for them.

What You Can Expect

Professional Expertise

Our SILC Support Coordination team brings specialist knowledge of the NDIS, disability supports, and service systems. The House Establishment Process explains how SILC helps bring these elements together to support a smooth transition into independent living.

Focus on Your Goals

We prioritise what matters to you and your family, not just what’s written in your plan.

Ongoing Support

We don’t just help you get started – we build your capacity consistently as we support you throughout your NDIS journey. It provides an opportunity to explore pathways, ask questions, and begin a conversation with SILC about next steps.

Responsive Communication

We’re available when you need us and keep you informed every step of the way.

Who We Support

Our Support Coordination services are designed for NDIS participants who:

  • Have NDIS funding for Support Coordination (Level 2 or Level 3) in their plan
  • Want guidance navigating the NDIS and connecting with providers
  • Wish to build their capacity to understand and manage their supports
  • Are experiencing barriers or complexity in implementing their plan
  • Value family involvement and collaborative decision-making
